E-mail: [email protected] INTRODUCTION It’s been over 50 years since the fi rst pacemakers were invented. Since then, important changes have been introduced concerning bradycardias, but also anti-tachycardia therapies such as anti-tachycardia pacing, internal defi brillation and devices aimed to improve synchronization in the failing heart. There is a dynamic regarding the indications for these therapies, which was imposed by the technical development of these incredible “mini”-computers, a process that still goes on today. Historically, there were experiments regarding cardiac resynchronization (CRT) starting with 1986 (Burkhoff et al) and also in 1990 (Latucca et al), by using animal models. Eight years later (1994) the fi rst CRT was surgically implanted in a human being – Serge Cazeau. In 1998 Daubert (et al) presented the technique using the coronary sinus and later in 2001

Cardiac Resynchronization Therapy in Women

Despite underrepresentation of women in cardiac resynchronization therapy studies, it has been conclusively shown that women demonstrate greater benefit than men in functional status, reverse cardiac remodeling, and survival and female gender remains an independent predictor of greater response to cardiac resynchronization therapy after adjustment for various other factors. Eligible female pati...

Heart failure monitoring with a cardiac resynchronization therapy device-based cardiac contractility sensor: a case series

INTRODUCTION The SonR signal has been shown to reflect cardiac contractility. It is recorded with an atrial lead connected to a cardiac resynchronization therapy defibrillator. For the first time, clinical evidence on the use of the SonR signal in the monitoring of the clinical status of heart failure patients implanted with cardiac resynchronization therapy defibrillator are presented through ...

Cardiac Resynchronization Therapy for Heart Failure

The weight of evidence supporting the routine use of cardiac resynchronization therapy, or atrial-synchronized biventricular pacing, as a treatment for patients with moderate-to-severe chronic systolic heart failure and ventricular dyssynchrony is now quite substantial. Results from mechanistic studies, observational evaluations, and randomized, controlled trials have consistently demonstrated ...

Cardiac resynchronization therapy for mild heart failure: the time has come.

Current guidelines recommend cardiac resynchronization therapy (CRT), previously known as biventricular pacing, in patients with left ventricular (LV) systolic dysfunction (ejection fraction [EF] 35%), QRS prolongation ( 120 ms), and New York Heart Association (NYHA) class III or IV heart failure (HF).1 These recommendations come after multiple prospective, randomized trials demonstrated the be...

Is Assessment of Systolic Dyssynchrony for Cardiac Resynchronization Therapy Clinically Useful? Assessment of Systolic Dyssynchrony for Cardiac Resynchronization Therapy Is Not Clinically Useful

Cardiac resynchronization therapy (CRT) is an effective treatment in patients with medically refractory heart failure. Several clinical trials have shown improvement in both left ventricular (LV) function and symptoms with CRT compared with controls,1–4 with the 2 largest trials showing a reduction in rates of hospitalization and death.5,6 On the basis of data from these studies, most internati...
